Reports to: QP and Director
Main Function: The intent of this service and position is to meet the habilitation and support needs of the individuals living in their own home or family’s home. Provide instruction and assistance to consumers who are at risk for out of home placement. Serves as a positive role model, mentor, and companion. Demonstrates knowledge, skills, and abilities required to serve clients based on the individual treatment and or behavioral plan.
Qualifications: A GED or High School Diploma. Must be at least 18 years old, able to read, write, follow directions, and comprehend. Must be able to complete core competencies (according to the plan of care) Must have a driver’s license and proof of liability insurance. Must have an approved criminal record and not be listed on the Health Care Registry. Must have an approved driving record.
Preferred: One year of documented experience preferred but not required working with population served.
General Duties/Responsibilities:
1. Provide instruction and assistance to enable the individual to acquire and maintain skills that will allow him/her to function with greater independence in the community and home setting.
2. Provide habilitation, training, and instruction coupled with elements of support, supervision, and engaging participation to reflect the natural flow of training, practice of skills, and other activities.
3. Works with QP and outside support staff to develop achievable goals for the individual served.
4. Keep the agency abreast of changing conditions and client outcomes.
5. Report incidents immediately to the QP or Director.
6. Attend meetings/staff development activities as required by the agency.
7. Interactions with the consumer in an effort to achieve outcomes designed in the ISP/PCP.
8. Training and/or support with socialization.
9. Training and/or support with personal skill development that includes activities designed to improve client’s own abilities.
10. Training and/or support with community participation, recreation, or leisure that includes the development or maintenance of skills to utilize community resources.
11. Activities-provides transportation for consumers as defined in the treatment/habilitation plan. Meet all applicable Liability Insurance Requirements.
12. Maintain a valid driver’s license that adheres to standard of employment.
13. Demonstrates knowledge of Majestic Support Services policies and procedures.
14. Complete documentation as required.
15. Accurately and correctly documents activities for placement in the clinical record.
16. Completes minimal training requirements. Attends all training sessions as required by the agency.
