Many times, this requires that we go outside and provide services in other settings to build the bridge that links individuals to our health center for preventive and primary care. Over 90,000 individuals come to La Clínica because it is a welcoming place that addresses the whole person, coordinating and connecting them to a broad network of services to improve and maintain their health and well-being.

About the Position: Under general supervision, will conduct nutrition screening, assessment and counseling a diverse population on various nutrition related diseases such as hypertension, high cholesterol, obesity, diabetes, as well as implement nutrition care plans for patients with diabetes, pre-diabetes, gestational diabetes and other high risk pregnancies, hypertension, obesity, and hyperlipidemia. Assist in curriculum and educational development and implementation of material, teaching classes, and quality improvement work.

Major Areas of Responsibility include but are not limited to:

  • Conducts nutrition screening, assessment and counseling, and implements nutrition care plans for low and high risk patients with diabetes, pre-diabetes, gestational diabetes, and other high risk pregnancies, hypertension, obesity, hyperlipidemia.
  • Provides individual nutrition and health counseling for those with overweight, diabetes, hypertension, and dyslipidemia.
  • Maintains documentation by charting SOAP notes in the progress notes.
  • May develop and participate in group nutrition education programs and medical group visits (i.e., diabetes, child obesity, dyslipidemia, hypertension).
  • Collects and reviews material for education such as pamphlets, audio-visual aids, books, and handouts. May assist in the development and evaluation of culturally relevant nutrition education materials.
  • Assists in data entry and retrieval and other quality improvement efforts for the department.
  • Promotes site based employee wellness efforts.
  • May provide in-service nutrition/health education to staff and community partners.
  • Participates in meetings related to nutrition, clinical health education, and wellness as required/assigned.
  • Other duties as assigned by supervisor.

Minimum Job Requirements

  • Requires completion of Dietetic and Food Administration Program from an accredited university; RD or RD eligible, plus one year experience working in a community health care setting. Masters in Nutrition/Dietetics preferred.
  • Must hold a current BLS (Basic Life Support) Certificate. You will have a 30-day grace period to obtain your certificate from the date of hire.
  • Knowledge of contemporary principles and practices of nutrition, nutritional value of foods, interpretation and implementation of special diets, and meal planning and preparation that is culturally appropriate.
  • Knowledge and experience with Motivational Interviewing, Stages of Change, Division of Responsibilities.
  • Interpretation of anthropometric, biochemical, clinical, and dietary information, and determination of appropriate dietary advice that is culturally appropriate.
  • Knowledge of aspects of storing and/or preparing foods properly; interpretation and implementation of special diets; scopes functions and responsibilities of local health departments.
  • Ability to develop, implement, and evaluate nutrition education activities and coordinate them with other departmental programs.
  • Ability to work across cultures and demonstrate support of diversity, equity and inclusion.
  • Bilingual English/Spanish required.
