We're seeking a skilled and empathetic Mental Health Practitioner to join our Assertive Community Treatment (ACT) team! This position supports Washington County and the 7E Region (Chisago, Isanti, Kanabec, Mille Lacs, Pine, and the Mille Lacs Band of Ojibwe).

What You'll Do:

As a Mental Health Practitioner on our ACT team, you'll provide trauma-informed treatment interventions that integrate clinical and psychiatric rehabilitation strategies. You'll guide person-centered planning, ensuring culturally congruent responses. You'll also help individuals access and maintain benefits and community resources, coordinate care with external providers, connect with natural supports, and provide crisis intervention services.

Collaborative Care: Work with the team to provide ACT services, taking primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in collaboration with the client and treatment team.
Comprehensive Support: Assist in developing comprehensive assessments, individualized treatment plans, substance abuse treatment plans, and other tools to support clients in the community.
Housing and Independent Living Skills: Assist clients with all aspects of psychiatric rehabilitation and obtaining and maintaining housing. Teach clients independent living skills, such as cooking, shopping, cleaning, personal hygiene, transportation, and budgeting. Help clients make and keep appointments and other tasks needed for recovery.
Vocational Support: Work with the vocational counselor to encourage competitive employment and assist in fostering positive work skills.
Legal Support: Attend commitment and other court hearings as required and complete necessary court reports. File required reports with relevant agencies when applicable.
Substance Abuse Treatment: Lead in assessing, planning, and treating substance abuse for assigned clients, both individually and in groups.
Family and Community Engagement: Actively and assertively engage with clients' families/significant others, landlords, and employers (with client permission).
Crisis Intervention: Perform shift management duties, provide on-call crisis intervention (including nighttime hours), and serve as a backup to evening and weekend staff on a rotating schedule. Be available for weekend and holiday duties on a rotating schedule.
Administrative Tasks: Attend and participate in all scheduled team meetings. Transport clients in the community. Complete all paperwork, reports, and charting promptly and in an organized manner.
Professional Development: Actively participate in ongoing professional growth and development, maintain appropriate professional behavior, and participate in appropriate supervision.
What You'll Need:

Education: Bachelor's or Master's Degree in Behavioral Sciences or a related field.
Experience: 2,000 hours of supervised experience working with individuals with mental illness.

Competitive Salary: $59,773 - $65,243 per year, plus a $50/month office benefit package (subject to change).
