About the Role:
As a Mental Health Practitioner on our ACT team, you will provide comprehensive, integrated care to adults living with mental illness, substance use disorders, and associated medical conditions. You will deliver services where and when clients need them most - in their homes, at work, and other community settings. You will work closely with clients to help them recover and respond quickly with additional support as needed. This position supports individuals in Washington County and the 7E Region (Chisago, Isanti, Kanabec, Mille Lacs, Pine, and the Mille Lacs Band of Ojibwe).
What You'll Do:
Provide trauma-informed treatment interventions that integrate clinical and psychiatric rehabilitation strategies.
Guide person-centered planning practices, assessing for and integrating culturally congruent responses.
Help people access and maintain benefits and community resources.
Coordinate care with external providers and connect clients with natural supports.
Provide crisis intervention services.
Collaborate with an interdisciplinary ACT team, including mental health professionals, psychiatric care providers, nurses, substance use specialists, vocational specialists, and certified peer specialists.
Develop, write, implement, evaluate, and revise overall treatment goals and plans in collaboration with the client and the treatment team.
Assist in developing comprehensive assessments, individualized treatment plans, substance abuse treatment plans, and other tools to support clients in the community.
Assist clients with all aspects of psychiatric rehabilitation and obtaining and maintaining housing.
Teach clients independent living skills (cooking, shopping, cleaning, personal hygiene, transportation, budgeting, etc.).
Support clients in making and keeping appointments and other tasks related to their recovery.
Work with the vocational counselor to encourage competitive employment and foster positive work skills.
Attend commitment and other court hearings as required and complete necessary court reports.
File mandated reports (vulnerable adult, child protection, ombudsman, etc.).
Lead in assessing, planning, and treating substance abuse for assigned clients (individually and in groups).
Engage with clients' family members/significant others, landlords, and employers (with client permission).
Perform shift management duties.
Provide on-call crisis intervention (nights, weekends, holidays on a rotating schedule).
Transport clients in the community.
Complete all paperwork, reports, and charting promptly and in an organized manner.
Actively participate in professional development and supervision.
Maintain regular work hours.
Perform other duties as assigned.
What You'll Bring:
Bachelor's or Master's degree in Behavioral Sciences or a related field.
2,000 hours of supervised experience working with individuals with mental illness.
