Forms Designer Location Syracuse, NY (University Hill area) : The Forms Designer coordinates the tracking and approval process for medical record forms. Syracuse Information Management & Technology Full-time Opening on: Jan 9 2024 IMT-Educational Communications State of New York Instructional Support Technician, NSSL3 79080 UUP (State University Professional Services Unit) Job Summary: The Forms Designer coordinates the tracking and approval process for medical record forms. This process includes working with department representatives who request a new form or wish to change an existing form. The designer keeps an inventory of all forms and tracks forms throughout the process. Forms must meet specific criteria, including appropriate reading levels and be approved by the Legal Department and are presented to the Forms Committee for final approval. The Designer will also assist in the design of web pages and assist the web team with design projects. Minimum Qualifications: Bachelor's degree in Graphic Design, Web Design, or related field and two years of experience designing forms for customer use or an Associate's Degree in Graphic Design, Web Design or related field and five years experience designing forms for customer use. Practical experience with Adobe InDesign and experience with Adobe Acrobat and Adobe Photoshop. Experience and proficiency with Microsoft Word, Excel and PowerPoint required. Must have demonstrated exceptional organizational skills. Preferred Qualifications: Bachelor's Degree preferred. Experience in web design in addition to forms design is a valuable asset. Experience with interactive .pdf files. Experience with HTML and CSS, and use of a content management system (CMS).
